You can download the new "CloseKnit Health" app on the App store or on Google Play and use the same username and password that you currently use to access primary care services.
You can also sign in by going to: http://portal.closeknit.com. If you need help doing this, you can reach out to techsupport@closeknit.com or to your care team via messages in the app CloseKnit patient portal or by call us at 866-233-6925.
Patients in all 50 states can use the new CloseKnit Health app to schedule primary care appointments.
